This involvement work is for patients registered at the above mentioned practices, including the Carlton Garden Surgery branch practice

Brief description of project

Representatives from Oakwood Surgery, Rutland Lodge Medical Practice (including the branch, Carlton Gardens Surgery), and Shadwell Medical Centre are currently in discussions about combining the three practices (a merger). They need to do things differently across these GP practices to ensure that the services are sustainable in the future.

The purpose of this involvement work is to inform patients, staff, and local people about these plans and to hear feedback about this proposal.

Who will we speak to?

We want to hear from patients, relatives, friends, or carers of patients registered at either Shadwell Medical Centre, Oakwood Surgery, or Rutland Lodge Medical Practice

What and how will we ask?

We want to hear from people about what is important to them when accessing primary care services and the thoughts they have on the proposals to merge Shadwell Medical Centre, Oakwood Surgery, and Rutland Lodge Medical Practice

People can give feedback using an online survey (link below), paper survey, attending one of three public events, or by contacting the practices directly. The three practices are writing to each household of registered patients with full details about this proposal and a survey to provide feedback

In-person meetings:

Tuesday 13 June, 1pm – 2pm – Shadwell Medical Centre (137 Shadwell Lane, Leeds, LS17 8AE)
Thursday 15 June, 2pm – 3pm – Rutland Lodge Medical Practice (Scott Hall Road, Chapel Allerton, Leeds, LS7 3DR)
Monday 19 June, 6:30pm – 7:30pm – Oakwood Surgery (Gledhow Rise, Roundhay, Oakwood, Leeds, LS8 4AA)

You don’t have to go to the practice you are registered at to attend, please come to the one that is easiest for you. They are open sessions, so you don’t need to book.

Partners and Practice Managers from our practices will be there to answer questions, as well as a representative from the ICB in Leeds.
