The Otley and Aireborough (O&A) LCP cover the footprint of Otley and areas situated in the Aire Valley, such as Yeadon, Guiseley, Rawdon and Bramhope.
Geographically, our footprint is quite large and is situated on the rural outskirts of Leeds. The partnership convened as a group prior to the arrival of a local care partnership and therefore has a history of building relationships across organisational boundaries. Following the arrival of a more formalised LCP, we are keen to build on these strong relationships and deliver true collaborative approaches to the health and wellbeing of their community. Current partners are represented from community NHS services, Leeds City Council, elected members, and third sector organisations. GP practices involved in our partnership are Aireborough Family Practice, Chevin Medical Practice, and Westgate Surgery – One Medicare.
Current priorities
We have a shared priority that focuses on community health and care services working better together for people living with dementia and their carers. This has involved scoping of current service provisions in the community and defining what ‘good should look like from a service to a personal perspective. We began to pilot how an annual dementia review could be delivered differently by piloting the review clinic at a local third sector organisation. This clinic coincided with the community café morning and had key partners in attendance (Carers Leeds, Alzheimer’s Society, and Memory Lane), so a warm introduction by the GP could be made where appropriate. This activity is currently on hold due to the recent COVID-19 disruptions.
We are currently taking part in wave 3 of the population health management programme for frailty. The population health management approach allows for more proactive and person-centred care planning methods when working with people. The partnership is using a data-driven approach to identify people living with frailty who have a diagnosis of dementia. Currently, the partnership is in the development stages of their localised intervention.
